interesting medieval surgery game (recommended for physicians)

So it was late one night and i decided to play some online games and I stumbled on this one.
It's very medieval and when i first saw it, it reminded me of the physicians and healers at kr.

I can log in, start the game, get a knife on my screen but cannot do anything.  What obvious component am I missing?

It's a good game. The second one is harder, and the third is just as good as the first.
